Social Worker Job vacancy

Family Support Team – Adult Inpatient Unit
30 hours per week to be worked over 4 days

£ per annum pro rata

Due to retirement we wish to appoint an experienced social worker to work within the existing multidisciplinary team. The successful candidate will work directly with patients with life limiting conditions as part of the multidisciplinary team utilising a broad range of social work skills.

This is a unique opportunity for a highly motivated person to join an innovative team who are committed to the delivery of Specialist Palliative care services in hospice and at home.

The successful applicant will have experience of working with adults who have complex needs, and will be required to build upon existing links within the community. Key responsibilities include discharge planning, psychosocial support for patients and families and safeguarding of vulnerable adults.

Experience in working with people with Palliative care needs would be an advantage. The successful applicant will be a highly motivated, empathetic and flexible person, with good interpersonal skills committed to multidisciplinary team working.
